Jenny Randerson has written to the Minister asking for an urgent investigation

Following the news that Hereford Hospitals NHS Trust has instigated an urgent review of Histopathology Samples, just weeks after a similar problem came to light in Wrexham, Welsh Liberal Democrat Health Spokesperson, Jenny Randerson AM has written to the Minister For Health asking her to investigate the cause of these serious problems and to issue new guidelines to NHS Trusts in Wales

Jenny Randerson said: "I understand that tragic mistakes do sometimes occur and can be explained by failures in internal management procedures. However two instances in such a short space of time gives serious cause for concern.

"I have today written to the Minister asking her to investigate the cause of these potential problems in the wider NHS and issue fresh guidelines to prevent any further instances in Wales."
